Transition Services

  •  Youth Transition services are activities that prepare students with disabilities to move from school to post-school life. We aim to gear our services to not only required transition services based on the student’s needs, but also those that are of interest, thus making it truly individual transition services. 
  • Community Transition services help individuals move from institutions back into their homes and integrate with the community in which they live.

Transition Services

The dRC’s Transition Services includes:

  • Assist people with disabilities to transition from nursing homes and other institutional settings back to their homes and community.
  • Provide, through the Youth Services program, information and training to youth and their families about transition to adult life.
  • Facilitate the SAIL (Self-Advocacy, Independent Living) program the dRC developed for youth ages 17-21.

All our programs and services are offered free of charge to our consumers. We rely on donations and grants for our operating expenses.
